Admissions Process

Embassy Healthcare’s admissions process is designed to help you and your loved one review options, determine service needs and define a care plan. Our staff will answer your questions and provide information about various senior care options at Embassy Healthcare facilities and in the surrounding community.
We’re committed to providing a seamless admissions process to help you and your loved one feel at ease during this time of transition. A Step-by-Step Guide to Our Admissions Process
Call the Desired Facility
You must contact a specific Embassy location directly if you or your loved one is interested in a facility. You will speak with an on-site admissions representative who can answer your questions and instruct you on the next steps if you wish to tour the community. Choose a time to visit the facility to learn about levels of care, room availability, costs, programs and amenities.

If the Future Resident is Hospitalized
If your loved one is currently in the hospital, please inform your admissions representative or have your social worker/case manager contact the Embassy community on your behalf. Understanding your loved one’s health status and clinical needs helps us determine if we can provide the necessary care and ensure a smooth transition from the hospital. We will remain in contact with you or your social worker/case manager to maintain consistent communication until your loved one is discharged to one of our communities.

If the Future Resident Is NOT Hospitalized
If your loved one is NOT currently in the hospital or at another senior care facility, he or she must see their physician and obtain a medical history and physical assessment within five days of admission. Those documents help our clinical staff prepare a personalized care plan to address your loved one’s unique needs.

Understanding Your Financing Options
If eligible, you or your loved one should apply for financial assistance (i.e. Medicaid or Medicare) before admission.
If you have already filed for Medicaid, please provide a copy of the application or your loved one’s Medicaid card. Our staff are happy to assist if you or your loved one need help applying for Medicaid.

Necessary Information and Documentation
Once you’ve decided to move into an Embassy community, you’ll need to provide a copy of the following at admission:
- Power of attorney form
- Living will
- Legal guardianship forms
- Insurance cards
- Medicaid card
- Social Security card
We will include those documents in your loved one’s file to ensure all their financial, legal and personal information is current and accurate.


What to Expect on Admission Day
We provide a move-in date and time to begin the welcome and check-in process. You’ll be greeted by our admissions staff and care coordinators who will ensure your paperwork, payment and insurance details are complete.
Our expert staff conducts a thorough health assessment and will review your medical condition, treatment prescriptions and medications. Afterward, you’ll be directed to your room where you can bring items and furniture from home to personalize your space.
You’ll receive a tour of the facility once you’ve settled in to help you orient yourself, learn about available amenities and meet staff. We’ll also ensure you understand safety and security protocols.
You may attend meals in dining areas and meet your neighbors or participate in orientation activities. Staff will conduct periodic check-ins to help you acclimate and answer questions.
