Heritage Rehab’s Physical, Occupational and Speech therapy services are also available to members in the community.

We are pleased to offer outpatient therapy services at several of our skilled nursing centers, where we can provide services to new clients or continue working with those who have recently discharged home.

Physical Therapy

Physical therapy focuses on strength, mobility, flexibility, movement and balance. You can enroll in physical therapy to help relieve chronic pain, recover from surgery or prevent rehospitalization. Your outpatient therapy program may include hydrotherapy, aerobic exercises, weight training and coordination exercises.

Occupational Therapy

Occupational therapy is intended to help you relearn or enhance your ability to complete activities of daily living. Those could include getting dressed, bathing, using the toilet and feeding. Practicing those “occupations” can promote independence and improve your quality of life. Outpatient occupational therapy also includes adaptive techniques to make tasks more manageable.

Cardiac Rehabilitation

Our outpatient therapy services include supervised cardiac rehabilitation to help you or your loved one recover from heart-related conditions and illnesses. Cardiac rehab can also help you manage the effects of chronic cardiac conditions such as coronary artery disease and heart failure. Embassy outpatient therapy includes structured exercise programs, heart health education and regular assessments to monitor your progress.

Education and Home Exercise Plans

The techniques you learn in outpatient therapy can often be done at home, too, to promote continuous recovery. Our therapists provide education on self-management techniques, activity modification and pain management strategies to encourage health literacy. They can also create home exercise plans to help you meet certain wellness goals.

The duration depends on the plan of care that is created by the therapist, although typical sessions are 30-60 minutes.

Does insurance cover the cost of outpatient therapy?

Most Medicare and commercial insurance companies offer some coverage of PT, OT, ST services. Our facility staff will help determine if your insurance is accepted and if you have any copays.

Do I need a referral to begin outpatient therapy?

A specific referral is not needed to compete an evaluation, however your physician will need to sign off.

What should I expect during my first session?

You’ll undergo an assessment, review your medical history and discuss what to expect from therapy. A therapist will help develop a personalized treatment plan and discuss how you’ll work toward your goals. You will schedule your next few sessions to ensure a consistent schedule.
